We are continently located in downtown Armonk and are easily accessible to Westchester county and Connecticut area.

NY is a direct access state. Physical therapists who are DPT can teat patient directly without a prescription for the first 30 days or 10 visits after which we will need a medical prescription from your doctor. Our office will help you with this.

On your first visit, your PT will perform a comprehensive evaluation which will include a detailed orthopedic assessment along with an external and internal pelvic floor muscle assessment. The results of this evaluation will help her create a tailored rehabilitation protocol which will optimize healing and expedite your recovery process.

Medical and surgical history reports, including results of any tests, performed such as MRI, x-ray, urodynamics study, defecography, etc.

List of medications (prescriptions and/or over-the-counter medication)

Referring doctor’s written a prescription for physical therapy. If you do not have one, you may print our copy here and bring it to your doctor for his/her signature.

Insurance card and a photo ID such as a driver’s license or passport.

Loose-fitting comfortable clothes. Comfortable footwear.

After the evaluation is completed, a detailed report is sent to your doctor. We believe in a team approach and we will make sure to update your doctor on your progress with PT.
